In this experimental study, changes in IOP and pupil were determined after the application of a mixture of aminoacid 10% L-arginine.HCl in 0.5% timoptol into the conjunctival sac of the left eye of five healthy albinotic rabbits (New Zealand White). The right eye was used as a control. The results proved that the mixture of 0.5% timoptol with 10% L-arginine.HCl significantly decreased IOP compared to the control eye during the entire experiment. The maximum decrease of IOP was reached in the 60th (20.1%; 4 torr) and 240th (10.7%; 2.1 torr) minutes. Compared with the effect of the mixture of both compartments, 10% L-arginine.HCl and 0.5% timoptol applied alone caused a non-significant decrease in IOP. Papillary diameters were identical in both eyes (7-7.5 mm) during the entire experiment. The authors suggest that the interaction of L-arginine.HCl with timoptol causes the formation of a new metabolite which is, in fact, already an effective component; this new substance decreases the production of aqueous humor in the corpus ciliare and, in this way, IOP is decreased in physiological conditions. Utilization of this knowledge in clinical conditions could contribute to the better utilization and more harmless use of antiglaucomatics in glaucoma therapy. For this reason, more long-term studies should be performed in order to evaluate the effect of this new metabolite applied locally. LA: Slovak
