Deep non-perforating sclerectomy

The French Journal of Ophthalmology of August/September 1999 presents a series of papers on perforating filtering surgery. The contributors are Baudouin, Mermoud, Bechetoille, Demailly and Sourdille. All presentations are in the French language. Baudouin states that in order to understand this type of surgery one has to understand the glaucomatous optic neuropathy which is the indirect consequence of a trabecular obstacle. As the resistance seems to be in the outer wall of Schlemms canal the so-called non-perforating filtering surgery is excellently suitable for a causal approach to the glaucomatous problem. It all seems to - down to the attack on the identifiable juxtacanalicular tissue. He feels that the difference between this procedure and trabeculectomy is in the first three months only. Mermoud presents in detail his technique of deep sclerectomy. He discusses the possible complications and post-operative treatment. Goniopuncture is often performed. Finally he feels that this procedure is also suitable for combined techniques. Bechetoille has a somewhat different technique which is well illustrated in color photographs. His way of removing the juxta canalicular tissue is by aspiration. In this series no pressure results are presented. Demailly also presents in photographs and drawing his technique and so does Sourdille. Of course Sourdille is an advocate of the use of a hydrogel implant. He presents results that the implants significantly improves the pressure control. The pressure level is about 2 mmHg lower in the implant group. LA: French
