Measuring structural changes in the optic nerve head and retinal nerve fibre layer

PURPOSE: Assessing structural changes in the optic nerve is fundamental in the diagnosis and follow-up of glaucoma. Many clinical decisions are based on determination of the stability or non-stability of glaucomatous damage. Retinal nerve fiber layer (RNFL) and optic disc photographs are very useful in clinics, but their assessment is based on subjective evaluation. METHODS: Confocal scanning laser ophthalmoscope and confocal scanning laser polarimetry are available, and allow objective and quantitative analysis of the optic nerve. These instruments are currently being assessed for their use in glaucoma follow-up. CONCLUSIONS: This article summarises their advantages and limitations in performing the difficult task of detecting progression or changes over time.
